Total Motile Sperm Count (TMSC)
TMSC refers to the number of sperms in a semen sample capable of swimming forward (motility) and thus reaching and fertilizing an egg. This parameter is a more comprehensive measure than total sperm count, as it focuses on the number of functionally viable sperm.
Why it matters: A higher TMSC means more sperms capable of fertilizing the egg are physically present. This increases the likelihood of pregnancies through natural conception and assisted ART like IVF and IUI.
- In natural conception, sperm must swim from the cervix to the fallopian tubes, where fertilization occurs. Only motile sperm can undertake this journey, so the more motile sperm available, the higher the chances of conception.
- In IVF, a higher TMSC improves the chances that the sperm used in fertilization will be active and viable. This is crucial for procedures like ICSI (Intracytoplasmic Sperm Injection), where a single sperm is injected into an egg.
- Clinical Importance: Studies such as those by Hamilton et al. (2015) have shown that TMSC predicts fertility better than the WHO criteria, as it focuses on sperm that are motile and capable of fertilization. This way, clinicians can make more accurate predictions about the likelihood of conception. Patients with a TMSC above critical thresholds are far more likely to achieve pregnancy, making it a key parameter to improve in any fertility treatment plan.
- The 5 million and 10 million TMSC thresholds are key in determining the suitability of ART. A TMSC below 5 million significantly reduces success rates for IUI, making IVF or Intracytoplasmic Sperm Injection (ICSI) more appropriate. At TMSC levels between 5–10 million, IUI may still be considered, but success rates improve above 10 million, making IUI a more viable option.
Sperm Morphology
Sperm morphology refers to the shape and structure of sperm cells. Sperm are considered morphologically normal with a smooth oval head, an intact midpiece, and a single, uncoiled tail. Abnormal morphology (e.g., double heads, bent tails) can prevent sperm from successfully swimming or fertilizing an egg.
Why it matters: Sperm morphology is a predictive measure of sperm quality and fertilization potential. Sperms with normal morphology are more likely to penetrate and fertilize the egg because their shape allows for effective swimming and successful fusion with the egg membrane.
In ART procedures like IVF or ICSI, sperm morphology directly impacts the quality of sperm selected for fertilization. A higher percentage of morphologically normal sperm increases the chances that selected sperm will result in viable embryos, thus increasing the pregnancy rate per transfer.
Clinical Importance: Edson et al. (2016) demonstrated the correlation between improved sperm morphology and higher pregnancy rates. Sperm morphology plays a key role in the overall fertility health of men. Morphology is crucial for ART treatments because it directly affects the sperm's ability to fertilize an egg. The more normal sperms there are, the higher the chances of success.
